Dr. Ross's Expert Review

Thorne 5-MTHF is the folate I use most often for methylation support. Folate is a B vitamin that feeds the methylation cycle, the chemistry your body uses to keep homocysteine in a healthy range and to run its detoxification reactions. The catch is that some people convert dietary folate and folic acid into the active form, L-5-methyltetrahydrofolate, slowly or not well at all. This product delivers folate already in that active form, so the body does not have to do the conversion first.

I reach for it in anyone who needs steady support for healthy methylation. That includes people working on healthy homocysteine balance, people building up their detox routine, and women planning pregnancy, since folate supports healthy fetal development of the brain and spinal cord.

There is real research behind active folate. In a randomized trial in healthy young women, Fohr and colleagues found that the homocysteine response depends on MTHFR genotype, and in a later double-blind trial Lamers and colleagues found that 5-MTHF lowered homocysteine as effectively as folic acid. Henderson and colleagues found in a 12 week trial that L-5-MTHF raised blood folate more than folic acid did, and Prinz-Langenohl and colleagues found plasma folate rose faster with 5-MTHF than with folic acid in women across MTHFR genotypes. The honest caveat is that response varies from person to person, and genotype testing can help guide the dose.

The practical side is simple. Start low and go slow, because sensitive people can feel overstimulated if methylation ramps up too fast, and some people pair it with vitamin B12 to keep the two in balance. If you take methotrexate, or you are pregnant or nursing, review this product with your treating physician first.

For a clean, hypoallergenic active folate that skips the conversion step, this is the one I use.

How I Use It in My Practice

I usually start with one 1 mg capsule once a day and give it a few weeks before judging it, moving up to two or three capsules a day, or to the 5 mg strength, when methylation demands are higher and my patient tolerates it well. I often pair it with methylated B12, since folate and B12 work together in the methylation cycle. In sensitive patients I start low and go slow, because ramping methylation up too quickly can cause irritability or restless sleep that settles once the dose is eased back. Anyone on methotrexate, or who is pregnant or nursing, reviews dose and timing with their treating physician first.

The Research, in Dr. Ross's Words

The research on 5-MTHF clusters around two questions: does the active form work as well as folic acid, and do genetics change the answer. In a placebo-controlled trial in healthy young women, Fohr and colleagues found that both 5-MTHF and folic acid lowered homocysteine, with the pattern of response shaped by MTHFR genotype. In a double-blind trial, Lamers and colleagues found that a daily dose of 5-MTHF reduced plasma homocysteine as effectively as folic acid in healthy women.

The absorption side favors the active form. Henderson and colleagues reported that 12 weeks of L-5-MTHF raised blood folate concentrations to a greater extent than folic acid in Malaysian women, with similar homocysteine lowering. Prinz-Langenohl and colleagues compared the two forms across MTHFR genotypes and found plasma folate rose faster with 5-MTHF. Willems and colleagues described how direct administration of 5-MTHF can facilitate the remethylation of homocysteine back to methionine.

Taken together, the trials support 5-MTHF as a reliable way to raise folate status, with homocysteine responses that vary with genotype and baseline status. The studies are mostly small and conducted in healthy volunteers, so I treat them as support for a well-understood mechanism rather than settled proof, and I individualize the dose.

This product is contraindicated in an individual with a history of hypersensitivity to any of its ingredients. If pregnant, consult your health-care practitioner before using this product. 5-methyltetrahydrofolate (5-MTHF) supplementation is not recommended concurrent with methotrexate cancer therapy, as it can interfere with methotrexate's anti-neoplastic activity; however, this folate source has not been shown to interfere with the anti-inflammatory activity of methotrexate. An individual taking methotrexate for psoriasis or rheumatoid arthritis can safely take a supplement containing folate.
